Custom form fields will only be displayed if a customer selects a service which the form has been applied too. For example, you can have a form for cleaning services, and a seperate form for other types of unrelated services, e.g. house washing, or gardening. If a customer selects a cleaning service, they will only see your cleaning services form and not unrelated questions.

If your customer selects two services in your booking form, and they both have the same custom form applied, the custom form will only present itself once in the booking process.
